Who We Are

Burlington Vestibular Therapy is a division of OMNI Health and Rehab, located at 960 Cumberland Ave, Burlington, ON L7N 3J6. We provide specialized vestibular physiotherapy services.

When you book an appointment or use our services, we may collect:

  • Contact information — name, phone number, email address, mailing address
  • Health information — medical history, symptoms, diagnoses, treatment notes, and assessment findings
  • Insurance information — provider name, policy number, group number
  • Appointment information — dates, times, services booked, cancellation history

How We Use Your Information

We use your personal health information to:

  • Provide vestibular therapy assessment and treatment
  • Communicate with you about appointments and care
  • Process insurance claims and direct billing
  • Comply with legal and regulatory requirements, including record retention
  • Coordinate care with other healthcare providers (with your consent)

Your Rights Under PHIPA

As an Ontario healthcare provider, we comply with the Personal Health Information Protection Act, 2004 (PHIPA). Under PHIPA, you have the right to:

  • Staff training on privacy obligations
  • Limiting access to information on a need-to-know basis

Information Sharing

We do not sell, rent, or trade your personal information. We may share your information only:

  • With your explicit consent
  • With other healthcare providers involved in your care (with your consent)
  • With your insurance company for billing purposes
  • As required by law (e.g., regulatory bodies, court orders)

Record Retention

We retain your health records for a minimum of 10 years from the date of your last visit, in accordance with Ontario regulations. Records of minors are retained for 10 years after the patient reaches the age of 18.

AI Chat Assistant

Our website uses an AI-powered chat assistant provided by Anthropic's Claude service to help answer questions about our clinic and services. When you use the chat assistant:

  • Your messages are transmitted to Anthropic for processing. Per Anthropic's Commercial Terms of Service, API conversations are not used for model training and data is deleted within 30 days
  • The assistant may collect your name, email, phone number, and message content if you choose to provide them
  • The assistant does not collect medical history, diagnoses, or clinical information
  • Conversation logs are retained for up to 90 days before deletion. Conversations are anonymized unless you provide identifying information

Third-party providers for chat:

  • Anthropic, PBC (San Francisco, CA) — AI processing
  • Fly.io, Inc. (Chicago, IL) — Hosting (deployed in Toronto, Canada)

Website Analytics

Our website may use analytics tools to understand how visitors use the site. This data is anonymized and does not include personal health information. We do not use cookies for advertising or tracking purposes.
